BIOFLEX Clinic Ambassador Club Official Policy & Participation Terms

Program Overview & Eligibility

The BIOFLEX Clinic Ambassador Club (“Program”) is a voluntary patient recognition initiative operated by BIOFLEX Laser Therapy Clinic (the “Clinic”).

Participation is available to all patients and was created to honour patients who trust our care, believe in their results, and choose to share that experience with others.

An Ideal Ambassador is an individual who:

• Has experienced meaningful healing and lasting results
• Has felt genuinely supported and cared for by the Clinic’s clinical team
• Values the Clinic’s standard of care and wishes for others to benefit from a similar experience
• Is able to communicate their experience openly, authentically, and responsibly

The Program is intended as a goodwill patient recognition initiative and not as a financial incentive for healthcare services. Participation does not create employment, partnership, agency, or professional affiliation and must not be interpreted as compensation for medical referrals.

As part of the Program, eligible participants may receive a preferred patient rate equivalent to a 10% reduction on standard Laser Therapy treatment fees. This preferred rate is not combinable with other discounts. This preferred rate is provided as a recognition benefit and does not constitute direct or indirect compensation, inducement, or consideration for patient referrals or healthcare services.

Participation in the Program does not guarantee eligibility for, or continued access to, any benefits, rewards, or referral privileges. Participation and continued eligibility, including the application of any Program-related benefits or considerations, are determined at the Clinic’s sole discretion. The Program remains voluntary and free of charge.

Referral Benefits

Eligible Ambassadors may share referral gift cards toward an individual’s first laser therapy treatment and receive one (1) Ambassador Point for each Successful Referral. Points may be redeemed for eligible clinic services, rentals, or BIOFLEX Systems, subject to availability and Clinic approval.

Ambassadors may hold a maximum of three (3) active referral gift cards at any given time. Additional cards may be issued as previously provided cards are shared with new patients, subject to the Clinic’s discretion.

Rewards are promotional, non-guaranteed, and subject to change or withdrawal at any time. Nothing in this Program guarantees medical outcomes or treatment results.

Successful Referral Definition

A “Successful Referral” is a new patient who has never previously received treatment at BIOFLEX Laser Therapy Clinic, presents a valid referral gift card, and attends and completes their first scheduled appointment, which must include both an initial consultation and a corresponding course of treatment. Completion of the appointment is subject to verification by the Clinic, including confirmation of attendance and receipt of treatment, as determined in the Clinic’s sole discretion.

Ambassador Points are issued only after verification. The Clinic reserves the right to determine, in its sole discretion, whether a referral qualifies as a Successful Referral.

Progressive Tier Reward Structure & Redemption

Ambassador Points are governed by a progressive tiered reward system, whereby greater point accumulations unlock enhanced total redemption value through structured bonus allocations.

Points may be redeemed only upon reaching a fully completed tier. Redemption values are determined strictly by the number of points redeemed in a single transaction and the corresponding tier outlined below:

3 Points: $150 base value + $25 bonus = $175 total value
5 Points: $250 base value + $50 bonus = $300 total value
10 Points: $500 base value + $150 bonus = $650 total value
15 Points: $750 base value + $300 bonus = $1,050 total value
20 Points: $1,000 base value + $500 bonus = $1,500 total value
25 Points: $1,250 base value + $750 bonus = $2,000 total value

Redemption must occur at a completed tier level; partial or incremental redemptions between tiers are not permitted. The total redemption value (including any applicable bonus) is available only when the full qualifying number of points for that tier is redeemed in a single transaction.

Points cannot be combined, divided, or applied in a manner intended to achieve a higher effective value than that assigned to the applicable completed tier. No bonus or enhanced valuation shall apply unless the full tier threshold is satisfied at the time of redemption.

Ambassador Points have no cash value, are non-transferable, and may be applied solely toward eligible clinic services, rental programs, or BIOFLEX Systems, subject to availability and Clinic approval. Points are not redeemable for cash, cash equivalents, or financial instruments of any kind. All determinations regarding tier qualification, redemption eligibility, and applicable rewards remain at the sole and absolute discretion of the Clinic.

Ambassador Conduct & Ethical Promotion

Ambassadors participate as independent individuals sharing personal experiences with the Clinic. Communications should remain respectful and aligned with clinical integrity, professionalism, and patient-centered care.

Ambassadors must not:

Present themselves as healthcare professionals or provide medical advice
Make misleading or exaggerated treatment claims
Engage in commercial promotion or solicitation
Pressure others to seek treatment
Use the Clinic’s brand in a manner that appears to be official advertising without approval

Participation may be suspended or terminated where conduct is inconsistent with Program intent or Clinic standards.

Tracking, Privacy & Administration

Referral activity is tracked within the Clinic’s secure system for Program administration. Personal health information is managed in accordance with applicable Canadian privacy legislation, including the Personal Health Information Protection Act (PHIPA), where applicable.

The Clinic is not responsible for lost, delayed, or inaccurately recorded referrals resulting from technical or administrative issues.

Program Changes, Points & Governing Law

The Clinic may modify, suspend, or discontinue the Program at any time. Ambassador Points have no cash value, are non-transferable, and do not constitute property or credit, and are not redeemable for cash, cash equivalents, or financial instruments of any kind.

If the Program ends, reasonable notice and a limited redemption period will be provided before points are forfeited. Participants are responsible for any applicable taxes.

This Program is governed by the laws of the Province of Ontario and the applicable laws of Canada.
